2007 AMBULANCE CLUB MEMBERSHIP

The Ickesburg Ambulance Association is participating in Third Party Billing. All insurances will be billed, including members. Any unpaid balance will be covered by your membership.

 

   Medically necessary transports are done only from Residence to Hospital, or Hospital to Residence. We Do Not transport between hospitals, Nursing  Homes or Doctor's Offices. All flowers and personal items must be taken home by the family.

 

   Mileage is computed on a round-trip basis from our Fire Station. The Ickesburg Ambulance is not responsible for any billing by the Advanced Life Support Services. All memberships must be received by December 31st, 2005. Any membership after that date will be subject to a 30-day waiting period.
